ACKNOWLEDGEMENT OF RISK: I, the undersigned, understand that participating in fitness classes and activities at Mind Body Fusion Fitness involves inherent risks, including but not limited to, physical injury, muscle strain, and other health complications. I acknowledge that I am physically fit and capable of participating in such activities.
RELEASE OF LIABILITY: In consideration of my participation in the fitness programs and activities provided by Mind Body Fusion Fitness, I hereby waive, release, and discharge Mind Body Fusion Fitness, its owners, instructors, employees, and agents from any and all liability for injuries, damages, or losses that may arise from my participation.
MEDICAL DISCLAIMER: I certify that I do not have any medical conditions that would preclude my participation in these activities. If I have any doubts about my health or ability to participate, I will consult a healthcare professional before engaging in any fitness activities.
INDEMNIFICATION: I agree to indemnify and hold harmless Mind Body Fusion Fitness from any claims, damages, or expenses, including legal fees, arising from my participation in the fitness programs and activities.
PHOTOGRPAHY & VIDEO RELEASE: I grant permission for Mind Body Fusion Fitness to take photographs and videos during classes and events, which may be used for promotional purposes.
